Boston University Brain Health Study

This page is to help Beatitudes Residents set up their Curana Portal.  This will allow you to receive surveys and questionnaires throughout the study period.  Do each step, IN ORDER, and then check off the box and move to the next step.  Varying from this process might mean failure and frustration.  If you need help, contact Portal@CuranaHealth.com or call them at 877-279-5960.

  1. [√] Sign up for the Study with Tena Alonzo.
  2. [_] Get your initial blood test.
  3. [_] Get your extra arm test
  4. [_] Get your extra arm test participation Gift Card
  5. [_] Send an eMail to Portal@CuranaHealth.com requesting a User Name and Password or a “One Time Code”
  6. [_] Sign up for your Curana Health Online Patient Portal
    • If you are familiar with QR codes, scan the code to the right.
    • CLICK THIS LINK this is the same link you will use throughout the Study.
    • If you received a User Name and Password, enter it, then click Sign In.
    • If you received a One Time Code, click on that link and enter your code
      • You will then be prompted to create a User Name and Password
  7. [_] You can use your laptop or your smartphone.
  8. [_] If using your smartphone, download the Netsmart myHealthPointe app.
  9. [_] If you are using your laptop or desktop,   CLICK THIS LINK.  It is the same one in Step 6.
  10. [_] Enter your User Name and Password, then click Sign In.
    • [_] If at any time you forget your password, click “Forgot Password.”
  11. [_] Contact Tena Alonzo (talonzo@beatitudescampus.org) to set up an appointment to connect their medical records to our study.
  12. [_] Complete your personal information profile
  13. [_] With your portal you can request appointments, send messages to your Care Team, get your personal health information, view medication, make refill requests, and more.  You can also participate in the Brain Health Study.

PLEASE NOTE:  The Patient Portal is for non-emergency use only.  If you are experiencing a medical emergency please dial 911 from your mobile phone or your Beatitudes phone.  If you use your Beatitudes phone, Security will automatically be notified and will also respond.  Security’s direct phone number is x16172 or 602-433-6172 on your mobile.  911 will NOT be notified if you call Security directly.
